The following steps describe the normal process for request and receipt of a QDRO
STEP 1 - To get started just fill out the QDRO Information Form completely (CLICK HERE TO VIEW AND PRINT). If you have an attorney representing you, make sure to fill out all the information about them on the form. If you do not have an attorney please leave this information blank.
STEP 2 - Fax, email, or mail the completed Information Form, along with a full and complete copy of your Final Court Order and/or Agreement to our offices.
STEP 3 - We will contact you to acknowledge receipt of your Information Form and call you if we need any additional information.
STEP 4 - Once we have all of the documents and information completed we will draft an Order specifically for your particular circumstances. We will provide it to you and, or your attorney. You or your attorney may call our offices at any time with any questions you may have.
STEP 5 - We will send the drafted and agreed upon Order to the correct Plan Administrator(s), for review and pre-approval.
STEP 6 - At this time the QDRO will be ready for you and/or your lawyer to present to the Judge in your jurisdiction for Order approval. You or your attorney will then send certified copies of the QDRO to the proper Plan Administrator(s) for implementation.
